What the Board needs
Tohaveyourcasereviewedyoumustcompletetheapplicationinthe
DSBbrochureandmailittotheaddressprovidedontheapplication
form.Somestateswillrequireyoutousecertifiedmail,withreturn
receiptrequested.
Yourapplicationisreviewedand,ifitisdeterminedtobeeligible,you
