How Do You Contact the Board?
Write to the Board at the follow ing ad dress to requ est a
brochu re/ ap plication. You will be sent a brochu re and a
one-p age custom er ap plication form . The form shou ld be
com pleted and mailed to the sam e ad dress.
What is the Review Process?
Your ap plication will be review ed and if it is determ ined to be
eligible, you will receive an acknow led gm ent ind icating the file
nu mber assigned to you r ap plication and the local Board
ad dress. At the sam e tim e, you r dealer and Ford Motor
Com pany rep resentative are asked to subm it statem ents.
To review you r case prop erly, the Board need s the follow ing
inform ation:

legible copies of all documents and maintenance or rep air
ord ers that relate to the case
■
the year, make, mod el, and vehicle identification nu mber
(VIN)
■

■
the date you bou ght you r vehicle
the date of rep air and the mileage at the tim e of rep air
the current mileage
the nam e of the dealer who sold you the vehicle or who
serviced you r vehicle
